Enlightened Minds Psychology Assessments is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information. We have adopted the Australian Privacy Principles contained in the Privacy Act 1988 (Cth).

Personal Information

Personal/client information is information or an opinion that identifies an individual. Enlightened Minds Psychology Assessments collects and holds the following personal/client information:

  • Your identifying information, such as name, address, and date of birth

  • Your contact information, such as phone number and email address

  • Identifying health information, such as your Medicare Care number and/or Private Health Insurance

  • Information about your records, such as past assessment reports

  • Information about your family and/or relatives

How Information is Collected

We collect your personal information for the primary purpose of providing our services to you. We will collect personal information about you in the following ways:

  • Directly from you, such as registration forms, online forms, Telehealth appointments, face-to-face appointments, via email, or over the phone

  • From a person responsible for you, such as a parent or guardian

  • From a third part when requires to or authorised by law

There may be times where we collect information for secondary purposes, including:

  • To help manage accounts and administrative services, such as billing, arrangements with Private Health Insurance, and management of file management software

When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

Sensitive Information

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual's racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information. Sensitive information will be used by us only:

  • For the primary purpose for which it was obtained

  • For a secondary purpose that is directly related to the primary purpose

  • With your consent; or where required or authorised by law

Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties, such as schools, teachers, and other allied health professionals. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party

Disclosure of Personal Information

Your Personal Information may be disclosed in a number of circumstances including the following:

  • Third parties where you consent to the use or disclosure

  • Where required or authorised by law

How your information is secured

Your personal information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure. This includes:

  • Storing information on a secure file management system

All Personal Information will be stored in client files which will be kept by us for a minimum of 7 years or until the client reaches the age of 25.

Disclosure of Information

Enlightened Minds Psychology Assessments may disclose information about you when you provide consent or when required by law. This may include:

  • To consult with other health professionals

  • To claim insurance

  • To communicate with Medicare or your Private Health Insurance

  • To help manage account and billing services

  • To comply with legal obligations, such as mandatory reporting, or respond to a court order or subpoena

  • To lessen or prevent a serious threat to your life or another’s life

Access to your Personal Information

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing. Enlightened Minds Psychology Assessments will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information. In order to protect your personal information we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important to us that your personal information is up to date. We will take reasonable steps to make sure that your personal information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.

Terms of Service

Fees

At Enlightened Minds Psychology Assessments, psychology services are billed at $230 an hour. This includes Telehealth appointments, face-to-face appointments, assessment report writing, and parent consultations. When booking an assessment, our psychologist will explain which assessment is required and discuss alternative pathways to ensure the best interests of the child.

You will receive an invoice for your appointment/assessment after your initial consultation with the psychologist. Payment can be made in instalments or all at once. However, we require full payment prior to your final feedback appointment. If full payment has not been received prior to your final feedback appointment, the feedback and report may be delayed.

Cancellation Policy

Enlightened Minds Psychology Assessments will charge a cancellation fee of $230 for appointments cancelled with less than 24 hours' notice.

Medicare, Private Health, and NDIS

Assessments cannot be claimed via Medicare. However, Medicare rebates are available for Autism Spectrum Disorder assessments when referred by a Paediatrician. In cases where there is a valid referral, we will provide you with all the information required to process a Medicare claim. However, we are unable to claim on your behalf.

Assessments may also be covered by your private health insurance. Similar to Medicare claiming, we will provide you with all the information required to obtain a refund/rebate. However, we are unable to claim on your behalf.

Assessments may also be covered by your child’s NDIS plan.

Please speak with your providers to determine what can be covered.

Mandatory Reporting

Psychologists are mandatory reporters and are required to report child abuse or neglect if there is a reasonable belief or suspicion. In these circumstances, they are legally obligated to disclose personal information and do not need to obtain consent.

Court Order

Enlightened Minds Psychology Assessments requires any Court Orders to be disclosed prior to beginning services.

Separated Parents

Enlightened Minds Psychology Assessments aims to uphold the child's best interests. We understand that parents may be separated, which sometimes leads to differing views on medical treatment. If parents are separated and a child is under 18 years old, legal consent will be sought from the parent requesting the psychological services. In addition, where possible, consent will also be sought from the child.

Enlightened Minds Psychology Assessments acknowledges that both parents have the right to request access to their child's medical records. However, there are circumstances when access to information may be refused, including:

  • There is a court order regarding custody and care arrangements.

  • There is a serious threat to an individual's life, health, or safety.

  • The child is a mature minor and can make decisions about their medical treatment.

  • Access would unreasonably impact an individual's privacy.

  • Access is not in the best interests of the child.
